  • Department: Mobile Crisis Services

  • Schedule: Full time, Monday - Friday, 9:00 a.m. - 5:30 p.m. and 1-2 on calls shifts a month

  • Facility: Ascension ProCare Center

  • Location: Melrose Park, Il

NOTE: To meet Medicaid Regulatory Guidelines associates must be at least 21 years old

Transportation: A personal vehicle is needed to conduct the field work in this role

Responsibilities

Perform activity based programs in support of patient care plan.

  • Conduct assessment of activity needs to meet individual treatment goals and objectives.

  • Facilitate and participate in activity-related projects and events.

  • Keep treatment area clean, safe, and organized.

  • Perform clerical tasks as assigned.

Requirements

Licensure / Certification / Registration:

  • Preferred Credential(s):• BLS Provider. American Heart Association or American Red Cross accepted.

Education:

  • High School diploma equivalency OR 1 year of applicable cumulative job specific experience required.

  • Note: Required professional licensure/certification can be used in lieu of education or experience,if applicable.

NOTE: To meet Medicaid Regulatory Guidelines associates must be at least 21 years old

To be successful in this role, we prefer that you have a personal life experience that includes having previously suffered from a mental health disorder and/or dual disorders of mental health and addiction disorder, received successful treatment, and has maintained stable recovery for a reasonable period of time.

Certification:

CRSS certification required within one year of employment

The Illinois Mental Health Collaborative for Access and Choice: Consumers: Certified Recovery Support Specialist (CRSS) Credential
